Advice for Accredited and Approved Programs

Dear Dr. NAACLS:
I have heard that it is possible to download various NAACLS forms from the
NAACLS website. I'd like to know how to do that so I can receive the forms even
if I'm not in my office and so I can fill in the forms electronically. Where are
they? Are there any forms that are not available?

The NAACLS web site has several commonly used forms available for download. From
the NAACLS home page,
www.naacls.org, there are a series of links along the left-hand side of the
page. Clicking on the appropriate program level will open the homepage for that
level. Clicking on "CLS/MT," for example, will open the homepage for Clinical
Laboratory Scientist/Medical Technologist.
The homepage for CLS/MT begins with contact information for the Discipline Lead
Person and the Program Services Coordinator. These are the people to contact as
you have questions or concerns during the review process. Below that, there are
links to download the Standards, Documents, the Guide, helpful matrices, and
other links. Many of the program homepages have matrices, which are helpful
tools to assist in compliance with curriculum standards. These are intended as
assistance, but are not required formats. The link to "Documents" will
open a page with numerous documents that can be accessed. There are three fields
to be completed. The first is the type of document you wish to access, the
second is the Title of the document, and the third is the format.
The document types and titles are very similar. Applications for continuing
accreditation or approval, the Guide to Accreditation or the Programs Approval
Guide, Position Change forms, Paper Review Report Forms, Site Visit Report
Forms, Standards and Clinical Affiliate Summary Sheets are all available. The
Fact Sheets (Sponsoring Institution, Faculty, and Clinical Facility) are
contained within the Guides. The File Format permits a choice between receiving
the document in Word or in Adobe .PDF formats. Please feel free to contact the
