The travel insurance company now requires:
(documentation below is also in the Volunteer manual)

- a Beneficiary Designation Request (BDR) form be completed by all volunteers doing business on behalf of NAACLS. Complete and return the form to your Program Services Coordinator by email or by fax, 773-714-8886 prior to traveling.

- NAACLS provide a MEDEX Insurance ID card to those travelling on NAACLS business
